DC Preschool Required Forms 2010-11

Acknowledgement of Receipt of Policies and Waiver Release

Please return this after you have reviewed the CommuniKids Preschool Parent Handbook.

Authorization for Medical Treatment

This form allows us to secure care for your child in case of an emergency.

DC Oral Health Certificate

The exam must have been given within the last year to be valid.

DC Universal Health Certificate

The exam must have been given within the last year to be valid.

Emergency Contact Form

This is CommuniKids' internal emergency contact form.

Medication Authorization Form

This form allows us to distribute medicine to your child if necessary.  If any medications are listed on this form, you must provide us with that medicine labeled clearly with your child's name and the dosage.

OSSE Registration Record for Children Receiving Care Away From Home

For DC licensing purposes.

Travel and Activity Authorization

This is a general form that gives us permission to take your child outside to play.  We will never take your child on a field trip without a specific permission form.
